Pulse of Change: A Black Health Initiative
Riverside Community Health Foundation proudly announces its collaboration with the California Health Care Foundation (CHCF) as a grant recipient of the CHCF’s Pulse of Change: A Black Health Initiative.
Alongside seven other distinguished organizations across California, Riverside Community Health Foundation has been selected to contribute insights aimed at enhancing the quality of California’s healthcare system through advancing Black health equity.
This partnership will focus on reducing hypertension rates among Black adults in Riverside County over the next two years. Strategies include promoting patient adherence to follow-up visits, ensuring timely care, and offering patient education services. Additionally, the initiative will provide:
• Free mobile blood pressure screenings
• Transportation support
• Referrals to culturally competent trained community health workers
By collaborating with CHCF, we aim to foster a healthcare environment where Black patients feel valued, respected, and empowered. Together, we are committed to building a safer and more resilient healthcare system for Black Californians.
