This vital information steers our approach to a healthier, happier, and more active community for generations to come.”
The Process
“RCHF is one of the largest and most important non-profit organizations serving the Inland Empire and the surrounding geographic areas of Riverside. As such, it will be important for RCHF to develop a resource allocation framework and process that ensures that its expanded scope is implemented effectively. This process revolves around our rich history of service and the Community Health Needs Assessment."

Timeline for the 2024 Community Health Needs Assessment
Stage 1: Assess
• Create public-facing information tools.
• Consistent communication with all stakeholders throughout the evaluation.
• Research the Service Area’s *Demographics
• Identify zip codes with high *socioeconomic need.
• Map and identify *assets and community strengths
• Compare *secondary data to other comparable locations, *Healthy People 2030, and historical data.
• Identify *disparities.
• Establish a method for analysis.
• Ensure active participation of partners, community leaders, and stakeholders in the design phase of analysis.
• Identify community input sources & choose a community input method.
• Draft a list of *significant health needs based on secondary data analysis and *community input.
• Complete *Community Context and Community Partners Assessments
• Reach out to partners and organizations to understand their resources within the service area.
Stage 2: Prioritize Health Needs
• Set criteria for prioritized health needs.
• Explore and choose method of prioritization.
• Summarize priorities selected.
Stage 3: Design
• Ensure the active participation of partners, community leaders, and stakeholders in the design phase of the implementation plan and evaluation framework.
• Choose significant needs to address.
• Justify significant needs you don’t plan to address.
• Set Goals.
• Select teams for each strategy.
• Research strategy options for best approach(es).
• Choose strategies & set objectives
